Donations to support the Hospice Program may be made by making checks payable to Hospice Care of Nantucket Foundation, and sending them to 57 Prospect street, Nantucket, MA 02554.

If you’d like your donation to be in honor of, or in memory of someone special, please indicate this and the person’s name, as well as the name and address of the person you’d like to be notified of this gift.

You may print and complete our donation form, or simply send us a note with the information.
